GLASS CLASS WAIVER & RELEASE
Center for the Arts Evergreen
I understand that participation in glass art classes involves the use of materials and tools that may present physical risks, including but not limited to:
- Cuts, burns, or injuries from glass, sharp edges, or broken pieces
- Use of hand tools and power tool and equipment that may cause injury
- Exposure to heat, dust, or small particles
I agree to follow all safety instructions, use safety equipment, and take responsibility for my own well-being while participating.
I voluntarily assume all risks associated with this class and release Center for the Arts Evergreen, its staff, instructors, board, and volunteers from any liability for injury, loss, or damage that may occur.
If registering a minor, I accept full responsibility for their safety and participation.
Photo & Video Release:
By registering for this class, I understand and agree that photos and video may be taken of me, my child, and artwork created during class. I grant permission for this content to be used for promotional, educational, and archival purposes.
If I do not wish to be photographed or recorded, I will notify staff in advance.
By signing, I confirm that I have read and agree to the above.
