Waiver and Acknowledgment: By registering for this class/camp, I, the undersigned, agree to the following terms and conditions for participation in the Summer Art Camp:

I understand that my child will be participating in art activities that may involve the use of tools, paints, clay, and other materials that could result in minor risks (e.g., stains on clothing or small scrapes from tools). I acknowledge these risks and agree to release the Center for the Arts Evergreen (CAE) staff, teachers, assistants, or board members from any liability related to these activities.

I agree to allow CAE to use any photographs or videos of my child and artwork created during the camp for promotional or marketing purposes.

I confirm that my child does not have any allergies or medical conditions that would prevent them from safely participating in camp activities, unless otherwise disclosed in this registration form.

I understand that CAE is not responsible for lost or stolen items or artwork during the camp.
Cancellation and Refund Policy (regardless of registration date):
- >2 weeks from the start of camp – $25 administrative fee per camp, per student - refund balance
- >1 week from the start of camp – $25 administrative fee per camp, per student - educational credit only.
- Within 1 week of the start of camp - no refunds, no transfers, no credits.
Please make sure you are able to attend all dates of the camp. No make-up classes or refunds will be offered for missed camp sessions.
In the event that CAE needs to cancel the camp due to unforeseen circumstances, parents will be notified via email and/or phone. Please provide contact information (email and phone) WHENEVER PROMPTED in registration forms.
If my child is dismissed from the camp for disciplinary reasons, no refund will be issued.
Acknowledgment and Signature: I have read and agree to the terms and conditions stated above. By registering for this class, I confirm my understanding and compliance with the cancellation and refund policy and all other policies.
